Overview

We are looking for a Licensed Masters Social Worker (LMSW) to join our team.

The Social Worker (LMSW) is a key member of the interdisciplinary team, primarily responsible for providing psychosocial support to patients and their families. This involves conducting assessments to identify needs, goals, interventions, and services in accordance with the established plan of care. The Social Worker utilizes professional training and personal judgment to monitor and support the psychosocial assessment process.

About You

• Master’s Degree in Social Work from a CSWE-accredited school
• State licensure as required
• Minimum one year of experience as a Social Worker in healthcare and/or hospice settings; three to five years preferred
• Knowledge of terminally ill patients and their families, understanding of hospice care, and the psychosocial dynamics of illness, loss, and death
• Ability to apply knowledge of hospice patient and family needs, demonstrate sensitivity to life and death issues, and support others during crises
• Capacity to cope with the stress of repeated loss
