Job summary

Agape Care Group is seeking a hospice medical social worker to conduct psychosocial evaluations and provide ongoing counseling for patients and their families at the end of life. You will support emotional well-being and collaborate with the care team according to the plan of care.

The role requires an MSW and active state licensure, with at least 2 years of clinical experience in healthcare or hospice, plus reliable transportation and the ability to lift up to 25 lbs.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• 2+ years of clinical work experience, preferably in healthcare or hospice
• Current state license as a social worker
• Reliable transportation and ability to sit, stand, bend, move intermittently and lift at least 25lbs

Responsibilities

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
