Agape Care Group is seeking a Social Worker (PT24) in the United States to support hospice patients and their families. You will perform psychosocial evaluations and provide ongoing counseling as part of the care plan, helping patients navigate end-of-life decisions with compassion and empathy.

The role emphasizes emotional support, collaboration with the care team, and commitment to a patient-centered approach in a supportive, values-driven environment.

Competenze

• A heart to serve patients and families and a passion for providing the best possible care
• Current state license as a social worker
• 2+ years of clinical work experience, preferably in healthcare or hospice
• Reliable transportation

As a hospice medical social worker, you will be responsible for psychosocial evaluations, and ongoing counse of patients and families during their end-of-life journey. Working in accordance with the plan of care, you will provide emotional support to patients and families when it's needed most.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
