Fort Duncan Regional Medical Center seeks an experienced hospital social worker to provide psychosocial support, discharge planning, and resource coordination for patients of all ages. The role supports families navigating complex care and ensures advocacy and smooth transitions from hospital to home or other settings.
The ideal candidate holds an MSW, licensure as a social worker, and strong English/Spanish communication skills, with a commitment to CQI and holistic patient care.
Responsible for provision of social work services and discharge planning to patients of all ages while they are in the hospital for acute, outpatient, or emergency and rehabilitive care. Interacts with interdisciplinary discharge planning team, medical, and nursing staff. Maintains performance improvement within the department and participates in C. Q. I. Provides crucial psychosocial support, helping patients and families cope with illness, navigate the complex healthcare system, and plan for discharge by assessing needs, offering counseling, arranging resources (like home care, financial aid), coordinating care with medical teams, and advocating for patient rights, ensuring holistic well-being alongside medical treatment.

Education: Master's in Social Work (MSW) from an accredited university or relatable degree. Licensure, Certification, Registration, etc.: Licensed Social Worker (M.S.W.). Special Skills, Knowledge: Ability to communicate effectively in English and Spanish preferred. Strong written and verbal skills. Basic computer knowledge. Provide care appropriate to the age of the patient served, pediatrics, adolescents, adults, and geriatric patients.
Three years of experience in a hospital setting preferred.
