Coordinates the discharge planning process, provides patient and family counseling, crisis intervention, financial needs assessment, information and referrals utilizing appropriate hospital department and/or community resources.

Qualifications

Masters degree in Social Work.

One year of social work experience preferred.

Texas licensed Master Social Worker.

Must have effective verbal/written communication skills and must have basic computer skills. Must demonstrate knowledge and skill in the evaluation of patient needs
