Job Description

AREAS OF RESPONSIBILITIES

Patients in need of Behavioral Health services.

PURPOSE OF POSITION

Under general supervision of the Director of Social Work and the Director of Mental Health Services, the Social Worker Level III provides:

SUMMARY OF DUTIES AND RESPONSIBILITIES

• All typical tasks described in the corporate social work job description.
• Individual and group psychotherapy
• IOP-specific Mental Health Screening
• Documentation and Paperwork:
  • IOP data tracking in database
• Timely patient notes
• Educative, didactic presentation where appropriate.
• Case management and concrete services.
• Liaison with other hospital services and units.
• Outreach to community resources.
• Interdisciplinary collaboration when necessary.
• Intake assessment and screening.
• Case presentations.
• Timely and accurate record keeping including treatment planning, progress notes and required correspondence for patient care.
• Participation in regular ongoing supervision.
• Make referrals to outside agencies.
• Crisis intervention.
• Home visits when appropriate.
• May supervise and help train other social work staff

Minimum Qualifications

For Assignment to Level III
1. Valid license as an LCSW issued by the NYSED.
