Wexner Medical Center seeks a Social Worker to provide psychosocial assessments, counseling, crisis intervention, and discharge planning for patients and families. The role links patients to community resources and supports safe transitions between care settings.
Minimum qualifications include an MSW and Ohio licensure (LSW/LISW) or equivalent. Background check is required; post-offer drug screen or physical may be needed.
Social work services include psyschosocial assessments, mental health counseling, crisis intervention, financial counseling, discharge planning, health education and the provision of material resources and linkage with community agencies. Social work services are provided to patients/families to meet their medically related social and emotional needs as they impact on their medical condition, treatment, recovery and safe transition from one care environment to another.
Core competencies include : Psychosocial assessment, counseling and crisis intervention skills, Knowledge of the PASAAR process for extended care facility placement and the assessment and counseling processes involved, knowledge of home healthcare set-up process and skills assessing neglect/abuse, including substance abuse and domestic violence.
Minimum Education Required Master's Level Degree Required Qualifications Master of Social Work from an accredited Graduate School of Social Work; State of Ohio Social Work licensure (LSW or LISW) or equivalent education/experience.
Final candidates are subject to successful completion of a background check. A drug screen or physical may be required during the post offer process.
