Job description

Overview

Provides social work services to patients and families within the Emergency Department to address psychosocial needs, enhance problem-solving and coping skills, and facilitate timely discharges by connecting patients and families to community resources. The social worker conducts psychosocial assessments, supportive counseling, trauma-informed care, and crisis intervention, collaborating with healthcare team members to ensure effective communication and care planning. This role includes field responses into the community, likely multiple times daily, to deliver mobile mental and behavioral health services within the C-U metro area, supporting the Illinois 988 response system in coordination with regional health and community agencies.

Responsibilities
  1. Support and plan for cases involving trauma, mental health crises, death, and fetal demise.
  2. Respond to community-level emergencies in partnership with EMS, providing mobile mental and behavioral health services.
  3. Respond to all levels of trauma, victims of violence, and mental health patients in the ED.
  4. Coordinate with security and spiritual care for next of kin notifications.
  5. Connect victims with community programs and resources, and communicate with outpatient social work to remove barriers.
  6. Collaborate on cases involving SANE patients, medication assistance, abuse/neglect, chemical dependency, guardianship, sex trafficking, and other social issues.
  7. Attend leadership and community meetings to strengthen community relationships.
  8. Evaluate patients for abuse/neglect, assist with mandated reporting, and collaborate with investigative bodies to ensure safety.
  9. Involve patients and families in case planning and keep management informed of complex cases.
  10. Provide counseling on advance directives, power of attorney, and conservatorship.
  11. Respond to referrals, complete psychosocial assessments, and identify patient needs.
  12. Assist patients in obtaining medications through assistance programs and arrange transportation as needed.
  13. Educate and connect patients with community resources for various social needs.
  14. Offer emotional support and empathic listening in person and over the phone.

Salary Range
