Responsibilities for a Social Worker:
  • Assist residents with emotional adjustment to the facility.
  • Interview new residents, record social history, and assessments as directed.
  • Complete the social services portion of the MDS, care planning, baseline assessments, etc.
  • Refer residents to psychological services as needed.
  • Schedule, attend, and participate in interdisciplinary care plan meetings, morning meetings, utilization review, etc.
  • Write summaries and progress notes as directed.
  • Assist in conducting pre-discharge evaluations, including setting up visiting nursing services and community resources.
  • Assist with discharge planning and follow up after discharge.
  • Conduct required Cut Letters.
  • Write discharge summaries as assigned.
  • Develop professional relationships with residents and their families.
  • Educate residents/representatives on Advanced Directives.
  • Adhere to all Social Services Regulations.
Qualifications for a Social Worker:
  • Minimum Bachelor's Degree in Social Work from an accredited college.
  • Must obtain a CSW and/or LSW.
  • One to three years of professional experience and/or training, or an equivalent combination of education and experience.
