The Social Worker is responsible for assessing, counseling, intervening, implementing and educating the patient, family/significant others to facilitate movement throughout the continuum of care.
Education: Bachelor’s degree
Experience: 2 – 4 years related experience
Skills: NJ Licensed Clinical Social Worker. Ability to communicate clearly in English, both orally and in writing. Knowledge of the discharge planning process. Knowledge of Medicare/Medicaid Regulations; community resources. Knowledge of JCAHO guidelines (continuation of care).
