The Social Worker LMSW or LCSW provides clinical social work services, including psychosocial assessments, discharge planning, and supporting patients and families through hospital experiences.

GENERAL DUTIES

1. Complete psychosocial assessments, develop plans, and implement interventions for patients with psychosocial risks.
2. Respond promptly to referrals based on urgency.
3. Assess psychosocial needs through interviews, team consultations, and record reviews.
4. Evaluate coping skills, support systems, and barriers affecting treatment and discharge.
5. Identify and plan interventions for barriers to care.
6. Communicate findings and plans with the interdisciplinary team and document in medical records.
7. Provide crisis intervention and short-term supportive counseling.
8. Assess suspected elder, child, sexual, or domestic abuse, and report as required.
9. Refer patients to community resources and facilitate interactions with agencies.
10. Participate in discharge planning, addressing complex social and medical issues.
11. Evaluate financial assistance needs and connect patients with resources.
12. Maintain knowledge of reimbursement requirements and community resources.
13. Address legal/ethical issues, including patient rights and advance directives.
14. Facilitate family meetings for decision-making support.
15. Link patients and families to ongoing support services like hospice and palliative care.

LICENSES AND CERTIFICATIONS

Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) through the Louisiana State Board of Social Work Examiners.
