Social Worker (LMSW) – Center for Advanced Digestive Care (CADC) – Outpatient – Full Time – Days

The Social Worker in the Center for Advanced Digestive Care (CADC) provides services to patients and families during some of the most challenging times in their lives. In this outpatient role, you will support patients living with chronic and complex gastrointestinal illnesses, helping them adjust to new diagnoses, navigate resources, and manage the impact of illness on their daily lives. You will connect patients with community resources, provide counseling, and facilitate support groups to address psychosocial needs.

This role provides coverage across multiple CADC programs—including surgery, oncology, and general gastroenterology—with a focus on 3–4 core programs while also responding to referrals from the broader center.

Schedule: Day shift, Monday–Friday

Preferred Criteria:

  • At least one year of medical/surgical inpatient experience
  • Experience in discharge planning

  • Master’s Degree in Social Work (MSW)
  • New York State License as a Licensed Master Social Worker (LMSW)
