Georgia Hospice Care is seeking a compassionate hospice medical social worker to join our team in Georgia. You will conduct psychosocial evaluations, provide ongoing counsel to patients and families, and deliver bereavement services while aligning with the plan of care.

You will also participate in community education outreach and support emotional and spiritual needs. Anticipate collaboration with a supportive leadership team and opportunities for professional development and advancement within

Qualifications

• MSW degree from an accredited school of social work approved by CSWE.
• Current state social work license.
• 2+ years of clinical work experience in healthcare or hospice.
• Reliable transportation and ability to lift up to 25 lbs.

Responsibilities

• Complete psychosocial evaluations and ongoing counsel to patients and families.
• Provide bereavement services and community education outreach.
• Support patients and families per the plan of care—physical, emotional and spiritual needs.

Qualifications

A heart to serve patients and families and a passion for providing the best possible care

• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25 lbs and bear the weight of an average adult effectively
