Job summary

The New Jewish Home's Adult Day Care Center at Waters Place in the Bronx is seeking a full-time Social Worker to join our team. This role focuses on interdisciplinary care planning and psychosocial assessments for clients in the Dementia Care Enhanced Memory Impaired program.

You will coordinate documentation, facilitate team and family meetings, and educate families about older adults' rights and well-being, while supporting the vulnerable elderly population.

Responsibilities

• Work as part of an interdisciplinary team to provide comprehensive MDS and psychosocial assessments for clients in our Dementia Care Enhanced Memory Impaired program.
• Coordinate and facilitate care planning and completion of all documentation
• Facilitate team and family meetings, education on older adults' rights and well-being for the vulnerable elderly population
