A leading mental health agency in Cleveland is seeking a Social Worker/Licensed Counselor to provide mental health services to at-risk youth. $50,000.00/yr - $62,500.00/yr

Qualifications:

• Bachelor’s Degree or higher
• LSW, LISW, LPC, LPCC, MFT or similar
• Desire to work a team approach with at-risk youth and families and value diversity and inclusion
• Ability to learn and use computer software applications
• Excellent verbal and written communication skills

Essential Responsibilities:

• Provide mental health services to at-risk children and adolescents with mental health diagnoses
• Develop client-focused treatment plans
• Complete daily progress notes and other Medicaid documentation within proper timeframes
• Triage and coordinate the mental health needs of clients and schools
• Provide individual and small group support for identified students.
• Support and coordination with families/guardians.
• Collaboration with community agencies on behalf of students or district program initiatives
• Consultation and coordination with school staff and administration on behalf of students
