ConcertoCare is seeking a Social Worker - LCSW to join our interdisciplinary team providing integrated medical, behavioral, and social care to patients with complex needs. The role is fully remote but must reside within the PST time zone, delivering virtual and telephonic visits in community settings.

The ideal candidate holds a Master’s in Social Work with licensure in CA or WA, has 2+ years of healthcare/behavioral health experience, and excels in care coordination, planning, and collaboration

Responsibilities

• Supports the behavioral health needs of patients on an assigned caseload.

• Participate in caseload reviews and clinical team meetings; communicate patient behavioral health needs effectively.

• Serve as a subject matter expert in behavioral health care within the team.
• Respect patient diversity in values, beliefs, and linguistic needs.
• Document encounters, referrals, and other pertinent information in the EHR.

The ConcertoCare Social Worker - LCSW is a core member of the interdisciplinary team responsible for providing integrated medical, behavioral, and social care to patients with complex healthcare needs.

Our Social Workers provide clinical care to medically and socially complex patients who live in community-based settings, including assisted living facilities, adult family homes, and private homes, through virtual and telephonic visits. This position ensures that the patient's biopsychosocial needs are adequately met through the completion of comprehensive behavioral health assessments and engaging the patient, caregiver(s), and care team in treatment planning and care coordination activities. The Social Worker is responsible for the provision of brief evidence-based behavioral health therapies and crisis intervention as a behavioral health expert within an integrated health care model. Social Workers also work collaboratively with other members of the local care team and is key in ensuring access to equitable behavioral health care in alignment with the organization's mission, vision, and values.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Supports the behavioral health needs of patients on an assigned patient caseload in accordance with the established clinical practices of the behavioral health team
• Uses evidence-based models of care to deliver brief treatment for common mental health issues such as depression, anxiety, coping with loss, mood management, and substance-use disorders
• Support psychotropic medication management by collaborating with prescribing providers, supporting treatment adherence needs, partnering with patients/caregivers for continuous monitoring of efficacy and potential negative side effects, and coordinating with internal psychiatrists and clinical care teams to address urgent psychotropic issues
• Participate in regularly scheduled caseload reviews and clinical team meetings; facilitate effective communication regarding patient's behavioral health needs among all members of the clinical team both verbally and in writing
• Complete and document assessment of patients in a timely manner
• Facilitate treatment plan changes for patients who are not demonstrating improvement in symptoms by consulting with patient's primary care provider/members of the clinical team and using measurement-based care to attune behavioral health interventions to the existing needs and symptom burden of patients
• Demonstrate the ability to serve as a subject matter expert in a particular area(s) of behavioral health care and supports the behavioral health team at large by sharing knowledge and resources related to area of expertise
• Ensure care is delivered in a manner that honors patient's diverse values, beliefs, behaviors, backgrounds, social, cultural, and linguistic needs
• Document patient encounters, referral activities, and other pertinent information in electronic health record ensuring all encounter notes are locked in accordance with company policy.

QUALIFICATIONS

• Master's degree in Social Work
• Current California or Washington independent licensure required
• 2+ years of prior healthcare / behavioral health experience required
• Strong problem-solving and organizational skills
• Geriatric care experience highly desired
• Ability to manage patient complexity and multiple clients with diverse needs
• Ability to communicate effectively in writing and verbally.
• Demonstrated ability to perform multiple concurrent tasks with minimal supervision and meet deadlines.
• Ability to work in a fast paced, dynamic environment and work well with others on a team.
• Proficient in computer skills to include Microsoft Office Suite (Outlook, Excel, PowerPoint, Word) and EMR experience
• Reliable internet connectivity required
• Knowledge and ability to navigate internet-based tools and applications, and proficient in computer documentation
• Demonstrates high level of professionalism.

Base Salary/ Wage Range $85,000 to $100,000 plus annual bonus. Compensation for the role is commensurate with the candidate's qualifications, skills, competencies, and experience and may fall outside of the range shown.
