HCA Florida North Florida Hospital is seeking a Social Worker for the Senior Healthcare Center in Melrose, FL. The role focuses on assisting patients and families with social and emotional issues impacting care, coordinating plans of care, system navigation, and referrals to community resources.
The position works with an inter-disciplinary team within outpatient Senior Healthcare Centers and may require travel to partner locations for meetings or events.
The Social Worker in the Senior Healthcare Center assists patients and families along the care continuum with social and emotional issues that may affect their medical care and well-being. The social Worker is responsible for case management, healthcare system navigation, as well as education and coordination of available and relevant community resources and referrals. Serves as a CMS-designated qualified health care provider completing services, documentation, and insurance-based requirements. Works with patients, families, physicians, staff, inter-hospital disciplinary team, and external community partners to facilitate plan of care.
This position is located at the Senior Healthcare Center Melrose office. Senior Healthcare Centers are outpatient departments of HCA Florida North Florida Hospital. Collaboration with other Senior Healthcare Centers and hospital department team members is anticipated. Travel to other company or community locations for meetings or events may occasionally be required.
