Overview
The Clover Care Services organization delivers proactive support and care to our members through our clinical Clover Home Care teams, and quality improvement services to our aligned providers through our practice engagement team. Position: Licensed Clinical Social Worker (LCSW). The LCSW provides behavioral health case management services, social services support, and advanced care planning guidance to appropriate patients in accordance with licensure, privileges, and scope of practice regulations.
Responsibilities
- Provide in-home and telehealth visits to patients. This position is primarily remote, with occasional in-home visits.
- Collaborate as a member of the Interdisciplinary Care Team (IDT) to provide behavioral health case management, social services support, and advanced care planning guidance to patients and caregivers.
- Provide behavioral health case management services, including:
- Comprehensive behavioral health assessments, mental status examinations, and diagnosis of Behavioral Health conditions.
- Development of behavioral health treatment plans that connect patients with medical, psychiatric, addiction, and other services as needed.
- Facilitation of linkages to psychiatrists, therapists, and community-based resources.
- Follow-up to ensure services align with individual care plans and adjust as needed.
- Development and execution of crisis management and safety plans, including acute crisis interventions.
- Provide social services support, including:
- Education about community resources, entitlement programs, and health care benefits to patients and IDT members.
- Advocacy to ensure access to care and resources; educate and encourage patient and caregiver advocacy to foster independence.
- Collaboration with social services agencies and other community organizations on behalf of patients and caregivers.
- Provide advanced care planning guidance, including:
- Education about health care proxies/durable power of attorney, living wills, and hospitalization decisions.
- Facilitation of completion of health care proxy/durable power of attorney, living will, and POLST forms.
- Working with IDT to translate prognosis into advanced care planning discussions.
- Deliver as-needed psychotherapy services using evidence-based modalities to support individualized mental health care and positive outcomes.
- Participate in ongoing professional development and self-improvement.
- Contribute to process improvement and clinical excellence initiatives as determined by management.
Qualifications
- Masters Degree in Social Work from an accredited program, required.
- Current and valid LCSW license in the State of New Jersey, required.
- Must communicate in English (oral and written); bilingual (Spanish) preferred.
- Valid New Jersey drivers license, required.
- Own reliable transportation to and from member homes, required.
- Three or more years of experience providing clinical services as an LCSW in inpatient or outpatient settings (inpatient and outpatient experience both preferred).
- Experience providing social services support; advanced care planning experience preferred.
- Competence in providing individual therapy.
- Knowledge of psychopathology, differential diagnosis, and treatment techniques.
- Excellent interpersonal skills and ability to communicate effectively with patients and colleagues.
- Basic computer proficiency; EMR experience (AthenaNet preferred).
- Comfort with stairs (at least 3 flights) as some residences may not have elevators.
- Commitment to working with patients from diverse social, ethnic, and cultural backgrounds.
