Position Summary
The Social Worker is responsible for the assessment, planning, negotiation, implementation, coordination, monitoring and evaluation of the patient’s plan of care from admission to post-discharge in collaboration with the interdisciplinary team (internal and external) to ensure a seamless, effective and efficient transition of care across the continuum. Adheres to NASW (National Association of Social Workers) Code of Ethics and exemplifies the qualities and values of the social work profession. The Social Worker promotes and protects patients’ needs while protecting patients’ rights to facilitate achievement of quality and cost effective patient outcomes. Has basic working knowledge of utilization management, payer requirements and specifications along with an in depth understanding of pre/post-acute resource availability. Participates in quality improvement and evaluation processes related to the management of care.

Qualifications
Education/Qualification
- Current Licensure in Social Worker (LGSW/LICSW) in the District of Columbia. LICSW preferred
- Master's degree in Social Work (MSW)
- Minimum of three (3) years’ experience plus two (2) years in case management required (Hospital setting preferable)
- Case Management Certification preferred
