Job Overview:

Provides behavioral health consultations and short-term therapy utilizing a team-based model in adult clinics to improve psychosocial functioning for adults and families. Responsibilities include providing education to Residents and Medical Staff, collaborating with healthcare teams and outside agencies, advocating for patients, conducting assessments, developing treatment plans, and maintaining documentation. Schedules appointments and maintains face-to-face client interactions as required.

Qualifications:

Required:

• Master's level education with 2 years of experience in human services
• Experience providing consultation and brief psychotherapy to adults in a medical setting (preferred)
• Familiarity with the Cherokee/Integrated Behavioral Health Model and knowledge of chronic diseases (preferred)
• Licensure: LCSW

Scheduling Requirements:

• 8:00 AM - 5:00 PM
• Days, Full-time

Essential Functions:

• Identify and facilitate clinically appropriate groups
• Develop case formulations and treatment strategies aligned with goals
• Track Performance Improvement Indicators and work towards benchmarks
• Maintain weekly schedule and documentation
• Maintain licenses and certifications
• Engage with agencies, schools, and families as appropriate
• Provide weekly productivity reports
• Refer patients to ensure continuum of care
• Provide psychotherapy, conduct intake assessments, and submit reports timely
• Participate in meetings supporting integrated care
• Evaluate crises and apply interventions
• Formulate diagnoses and make treatment recommendations
• Consult during primary care visits using the Cherokee Model
• Screen patients for psychosocial and behavioral needs
