TOUR 2 (9am-5pm)

Purpose of Position:

To provide and collaborate with internal and external sources to provide comprehensive services to homeless safety net patients.

Summary of Duties and Responsibilities:

Under the supervision of the Director for Social Work Services, Associate Director, or Social Worker Level V:

  • Complete biopsychosocial assessments per referral and assessment policy, including a holistic history of homelessness through review of EPIC and other relevant databases. Plan and carry out appropriate interventions reflective of trauma-informed principles.
  • Provide ongoing supportive counseling to patients, families/significant others, as well as concrete services.
  • Develop and implement treatment plans appropriate to patient needs based on assessments.
  • Understand and learn about the landscape of homelessness in the New York City metro area, including the Department of Homeless Services (DHS) shelter system, migrant shelter system, street outreach, housing landscape, and outpatient behavioral health services such as Assertive Community Treatment (ACT)/Intensive Mobile Treatment (IMT) teams.
  • Coordinate with the housing navigation team by reviewing the General Housing Intake Tracker (GHIT), monitoring patient referrals and progress, submitting supportive housing applications (2010E), and scheduling conferences as needed.
  • Complete referrals to Crisis Respite, Medical Respite, Joint Command Center (JCC) requests, Single Point of Access (SPOA) applications, Safe Options Support (SOS) referrals, and others when appropriate.
  • Coordinate with external care teams, such as ACT/IMT teams, shelter case management teams, SOS teams, etc., when appropriate.
  • Collaborate with inpatient teams in care planning and transitional care for patients attributed to the Safety Net Clinic or Street Outreach and Wellness (SHOW) panel.
  • Work with team members such as the SHOW Social Worker and Safety Net Clinic Community Health Worker (CHW) teams.
  • Attend Social Work meetings, individual, unit, and departmental hospital meetings as mandated.
  • Participate in supervisory conferences with social work supervisors.
  • Provide supervision and mentoring to assigned social work staff, including onboarding, training, documentation review, and complex care coordination support.
  • Complete performance appraisals for assigned staff.
  • Ensure timely and accurate documentation as per policies.
  • Participate in the department’s cross-training and coverage programs.
Minimum Qualifications

For Assignment to Level III:
1. Valid license as an LCSW issued by the NYSED.
