Job Summary:

Provides a wide range of clinical social work services in a primary care or home health setting, including support and consultation to care team in managing patients. Evaluates psychosocial assets and deficits to enable patients to maintain the greatest level of independence. Coordinates with others to establish a plan of care. Provides short-term counseling, teaching, and resource brokering for caregiving needs.

The Social Worker Mental Health Specialty case management position acts as a clinical resource within the Mental Health Specialty clinics, actively participating as a member of the care team. The role involves participating in clinic huddles, identifying patients with severe persistent mental illness (SPMI) who could benefit from additional support, and providing both virtual and in-person care in individual and group formats. This hybrid role combines therapeutic intervention with community connection support, addressing both acute and chronic mental health needs.

Essential Responsibilities:

  • Provide appropriate social work interventions for patients of all ages and their families, including psychosocial assessment, brief individual or family therapy, advocacy, linkage, referrals, group work, case coordination, and long-term care planning and placement.
  • Consult and coordinate with a multidisciplinary team of care professionals and primary care staff, including family and care conferences, team meetings, etc.
  • Assess patient and family needs for care, communicate with healthcare providers, and collect relevant data throughout the care process. Evaluate psychosocial assets and deficits to support patient independence.
  • Evaluate patient and family needs through timely assessments, communication, and observation, and modify care plans based on patient response and system feedback.

Basic Qualifications:

  • Minimum one (1) year of previous social work experience. (Note: Current KP Coalition employees are exempt per the National Agreement.)
  • Master's degree in Behavioral Science.

License, Certification, Registration:

  • Mental Health Counselor License (Washington), OR Licensed Marriage and Family Therapist (Washington), OR Licensed Advanced Social Worker License (Washington), OR Licensed Independent Clinical Social Worker (Washington).

Additional Requirements:

  • Excellent written and verbal communication skills.
  • Proven success working within a multidisciplinary team and a commitment to diversity.

Preferred Qualifications:

  • Two (2) years of post-master's experience.
  • Experience in care coordination within managed care systems, inpatient/outpatient settings, skilled nursing, or long-term care facilities.
  • Knowledge of social case management, conflict resolution, and team building practices.
  • Familiarity with Microsoft Office and electronic health record systems.
