Job Summary:

Provides short-term counseling, teaching, and resource brokering for caregiving needs.
The Social Worker Mental Health Specialty case management position acts as a clinical resource within the Mental Health Specialty clinics, actively participating as a member of the care team. The role involves participating in clinic huddles, identifying patients with severe persistent mental illness (SPMI) who could benefit from additional support, and providing both virtual and in-person care in individual and group formats. This hybrid role combines therapeutic intervention with community connection support, addressing both acute and chronic mental health needs.
