Job summary

Tenet Healthcare in West Palm Beach is seeking a Per Diem Social Worker to coordinate care along the patient journey, assess transition needs, and promote safe, timely discharge.

You will conduct complex psycho-social assessments to prevent readmissions, identify patients at risk, and support resource utilization within available means.

Requires MSW; LCSW preferred; ACM preferred. Five years acute hospital social work experience; three years acute care. Active LCSW license.

Qualifications

• MSW required; LCSW preferred; ACM preferred.
• Two years of progressive experience in the department; five years acute hospital social work experience; three years in acute care.
• Active LCSW license required.

Per Diem Days

The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge and prevent avoidable readmissions.

Education

MSW required. LCSW preferred. Accredited Case Manager (ACM) preferred.

Experience

Required: Two (2) years department progressive experience. Five (5) years acute hospital social work experience. Three (3) years as acute care experience

License

BLS (AMERICAN HEART ASSN)

Training

Must complete and demonstrate competency in using the Tenet Case Management documentation system within 30 days of hire.
