This includes social work assessment, interventions and treatment plans, supportive counseling and crisis intervention, care management, discharge/transition planning, advocacy and support. Seeking resources and services to address unmet needs. The social worker also provides consultation services and in-service education programs related to the psychosocial aspects of health care to other professional staff members on the healthcare team. The Social Worker III will also provide clinical supervision/mentoring (independent from administrative supervisor provided by HHS leaders) to LSWs or LGSWs for onsite work as they pursue independent licensure with the board of social work.RESPONSIBILITIESIdentifies patients who require social work assessment and intervention through high-risk screening and referralsInterview clients and perform bio-psychosocial assessment to determine problems and appropriate types and methods of service/treatment. Complete necessary documentation for accurate and timely billing of clinical services providedDesign and follow through on intensive long or short-term service/treatment/care plans including collaborating and communicating with the interdisciplinary team, community and healthcare resources to move the plan of care forwardMay provide supportive counseling and crisis intervention using social work methodsProvide a range of social work interventions that include preventive and direct services, care management, advocacy and assistance to clients who have mental, emotional, or physical disabilities and other cases involving complex social and financial needsMay carry a caseload for continuing service/treatment and provide care coordination including discharge/transition planningInterpret and explain policies, regulations, and programs to clients and the publicRefer clients to appropriate community services, provide support and assistance in connecting to services as needed, and interpret social and emotional factors to others involved in treatment of clientsMay make field visits to clients' homes in departments/areas where trained and asked to do soAdvocates for patients and families by supporting patient rights, ethical decision making and accessing protective services (including assuring applicable reporting of vulnerability or suspected abuse as mandated by law).
