Position Overview

The Division of Mental Health is seeking a Social Worker II for the Chicago-Read Mental Health Center located in Chicago, Illinois. The role involves providing professional social work services to mentally ill patients, including assessment, treatment, court report writing, case management, discharge planning, and acting as a qualified examiner. The position ensures compliance with standards from the Joint Commission, Central Management Services (CMS), and other agencies. The Social Worker may also serve on departmental or hospital committees as needed.

Essential Functions
  • Perform professional social work services for mentally ill patients on an assigned unit.
  • Provide case management, including funding applications, psychoeducation, acting as a liaison with community agencies, and advocating for patients.
  • Conduct patient therapy, family therapy, and group therapies according to the multidisciplinary treatment plan.
  • Take a principal role in the discharge planning process.
  • Participate in departmental or hospital committees as necessary.
  • Perform other duties within the scope of the role as assigned.
