Job summary

Baptist Health in Jacksonville seeks a Master’s Level Social Worker (Social Worker II) on a PRN, day shift basis at Baptist South. You will be part of an interdisciplinary care team to address patients’ psychosocial needs and support their emotional well-being.

This role focuses on reducing barriers to care, providing psychoeducation to patients and families, coordinating care, and connecting individuals with community resources and referrals.
