Position Summary

The Social Worker facilitates care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate resource utilization. Responsibilities include assessing patients' transition needs, conducting psycho-social assessments, promoting safe discharge, and preventing readmissions. The role involves adhering to national case management standards, providing education, collaborating with healthcare teams, documenting accurately, and leading patient populations or teams.

Responsibilities
  • Conduct psycho-social transition planning assessments and interventions
  • Assist with cases involving adoption, abuse, neglect, including referrals to agencies
  • Coordinate care and oversee transition plans
  • Lead multidisciplinary patient care conferences
  • Make referrals, communicate with patients and families, collaborate with staff
  • Ensure patient education for post-acute needs
  • Complete documentation thoroughly in the Case Management system
  • Maintain accurate patient demographic and insurance data
  • Precept new staff and serve as a resource
  • Participate in quality improvement initiatives
  • Perform other duties as assigned
Qualifications

Education: Master's of Social Work (Required)

Experience: Preferred: 2 years in acute hospital settings

Certifications: Must be licensed or license-eligible as LICSW, LCSW, or LMHC, per state regulations.
