Licensed Clinical Social Worker - Palliative Care, Mid Coast Region

For a limited time, MaineHealth is offering up to a $5,000 Sign-on bonus for all eligible Social Workers!

Current MaineHealth employee members are ineligible; former members are ineligible until more than 3 months of separation from employment. Bonuses may be prorated based on schedule.

Maine state licensure as a Licensed Clinical Social Worker (LCSW) is required.

This role primarily serves patients in MaineHealth's Mid Coast Region in their homes, via phone, or through telehealth. It may also involve providing care to patients and families in the local palliative care clinic.

Position Summary

The Palliative Care Social Worker is a vital member of the interdisciplinary team delivering patient- and family-centered care to individuals of all ages living with serious illness. The goal of palliative care is to maximize quality of life and align medical care with patient preferences. Responsibilities include evaluating psychosocial needs, providing counseling and anticipatory grief support, facilitating family meetings, making community resource referrals, and collaborating with the care team. This position supports patients in the Mid Coast area and the larger MaineHealth Home-Based Palliative Care team.

The role involves evaluating psychosocial needs and providing diagnostic, preventive, and treatment services, including individual, family, and group therapy. It includes discharge planning, coordinating aftercare, and referring patients to community services. The employee collaborates with health and mental health professionals and offers consultation regarding social and environmental needs, working under general supervision in accordance with policies.

The Social Worker II is responsible for assessment and provision of therapy services, discharge planning, and community referrals.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  • Education: Master's Degree in Social Work/Clinical Counseling from an accredited institution.
  • License/Certifications: Valid state license as an LCSW or LMSW-CC.
  • Experience: Two years of relevant health or mental health experience preferred; competency in essential social work functions required.
