Job description

This position works for Hospice of Cincinnati's/Hospice of Hamilton’s North Regional Team - Monday-Friday days. The average patient caseload tends to be between 30-35 patients. On-call coverage is for the entire HOC service area, including occasional weekends and holidays. Mileage is reimbursed. Flexibility is required to support patients at all levels of care, including long-term care facilities and Inpatient Care Centers. Support to team members/staff is also expected as needed and desired.

Job Overview:

Assess the psychosocial needs of patients and families to develop, communicate, and implement effective plans for psychosocial interventions and complex discharges, including readmissions in high-risk populations. Serve as a resource regarding social systems, institutions, and behavioral and relationship patterns. Apply appropriate interventions skillfully to meet patient and family needs. Collaborate with the multidisciplinary team to ensure effective, efficient, and appropriate care, optimizing health status and ensuring smooth transitions between levels of care. Provide education and expertise to promote social work practice and profession within the hospital. Serve on specialty areas requiring an MSW for regulatory or grant funding purposes.

Job Requirements:
  • Master's Degree in Social Science or Social Work (Required)
  • Up to 1 year of Clinical Healthcare or Community Social Work experience (Required)
  • Computer literate
  • Experience in grief and bereavement counseling, alcohol and drug intervention, and support for victims of abuse, neglect, and violence
  • Knowledge of payer rules for reimbursement and community resources for underserved populations
Job Responsibilities:
  • Apply root cause analysis to readmissions and collaborate with healthcare team, patient, and family to resolve issues
  • Communicate (verbally and in writing) and collaborate with the multidisciplinary team to advise on plans and patient responses
  • Counsel other caregivers on techniques for effective psychosocial intervention and communication
  • Develop and implement care plans based on assessments, including counseling or crisis intervention, and facilitate linkages to community resources and services. Revise plans based on patient and family responses
  • Promote the social work profession through formal education and teaching rounds; serve as a resource and preceptor for new staff
  • Provide on-call services according to departmental policy, including timely response and follow-up
  • Respond to referrals and assess psychosocial and complex discharge needs using data from medical records, patients, families, and the multidisciplinary team
