For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.

The Clinics provide psychotherapeutic and psychopharmacological therapy, as well as individual, group, family therapy, crisis intervention, medication management, case management and collateral sessions. Our staff members follow an approach to health care that emphasizes wellness, recovery, trauma-informed care, and physical-behavioral health integration.

STATEMENT OF THE JOB:

The Agency has embraced the concept of a fully integrated system of care for all consumers of service. This requires that staff assess the client’s needs holistically and see it as their responsibility to connect the client with services needed both internal and external to the Agency, regardless of the door through which the consumer enters the system. Additionally, CCNS has a goal of providing age-appropriate services to individuals of all ages, and our Certified Community Behavioral Health Clinic (CCBHC) has a specific focus on serving older adults living with mental health and substance use conditions through treatment and support services.

The Social Worker I – OA will perform initial interview, crisis intervention, and ongoing therapy with individuals, groups and families, with a specific focus on serving individuals who are considered older adults (which will in general include individuals over the age of 55 years, but services will be based on each person’s functional age rather than chronological age). The Social Worker I – OA will adhere to all standards of contract compliance, audit readiness, Agency policies and procedures, and all other regulatory requirements. The Social Worker I – OA will work as a member of a team to help ensure the overall integrity and welfare of the Agency.

  • Meet Agency requirements for qualitative and quantitative contacts with clients. Includes conducting intake interviews, rendering diagnoses in accordance with DSM criteria, providing crisis intervention, providing psychotherapy in various treatment modalities including group work, conducting telephone referrals, providing advocacy, case management and referrals.
  • Meet Agency requirements for qualitative and quantitative documentation. Includes writing case notes, treatment plans, intake assessments, correspondence regarding clients, termination summaries, encounter forms and any and all other required documentation.
  • Ensure that referrals are made to appropriate internal and external services as required to meet the needs of the consumer and their families.
  • Ensuring that target population has access to service according to contractual requirements and Agency Mission.
  • Conduct individual, family, marital and group therapy as dictated by the clinical needs of the client and the program.
  • Monitoring and coordinating psychiatrist psychopharmacology appointments.
  • Responsible for all administrative and clinical documentation associated with direct service clients.
  • Maintain content and format of casework record according to regulatory guidelines.
  • Complete and submit all documentation required for Agency reimbursement for services in a thorough, complete, accurate and timely fashion in order to ensure both the smooth delivery of service as well as fiscal viability of the Agency.
  • Provide and/or participate in trainings related to behavioral health services.

QUALIFICATIONS:

  • Master’s Degree in Social Work from a nationally accredited institution.
  • NYS LCSW license required.
  • Obtain NPI (LCSW) and Medicare Provider (LCSW) number immediately upon employment.
  • 1-2 years of experience serving older adults in a clinical setting preferred.
  • Able to travel to client’s homes, meetings, conferences, and other locations for training.
  • Ability to lift to 10lbs.
