Job summary

Stearns County seeks a Social Worker to provide assessment and case management services for individuals and families within the public service framework.

Qualifications

• Bachelor’s degree in social work or closely related field.
• One year of progressively related experience involving the well-being of others OR completion of a County Social Work Internship.
• A valid Minnesota Driver’s License or ability to obtain within 30 days of employment.
• Ability to travel to multiple locations around Stearns County, some not accessible by public transportation.

Responsibilities

• Completes assessments of individuals and families to develop and maintain service plans.
• Maintains complete, accurate client records to document case activity.
• Coordinates emergency situations and placements outside the home as needed.
• Assesses and refers individuals, families, and possibly children to community services.
• Drafts materials for referral and review by the County Attorney’s Office; testifies in court as necessary.
• Conducts client visits at homes and offices.
• Collaborates with department and community service providers.
• Advocates for client needs within the department framework.
• Monitors progress toward service goals; participates in committee work.
