Job summary

Crescent Hospice is seeking a Licensed Master Social Worker to provide compassionate social work services to hospice patients and families in the Anderson area. This per diem role focuses on counseling, care coordination, and emotional support delivered in partnership with the interdisciplinary team.

As part of our mission-driven care, you will collaborate with the RN Case Manager and Bereavement Coordinator to create individualized plans that respect dignity, comfort, and human connection

Qualifications

• Master of Social Work from an accredited school of social work.
• Licensed as a Social Worker in the state.
• Driver's license, vehicle insurance, and access to a dependable vehicle.

Responsibilities

• Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement personalized social work plans and collaborate with the Bereavement Coordinator.
• Shape care plans and participate in interdisciplinary meetings.
• Provide empathetic counseling to patients and families.

• Provide Individualized Care Advocacy: Implement personalized social work plans and collaborate with your Bereavement Coordinator.

Qualifications
Education

• Required: Master of Social Work from an accredited school of social work

License Requirements

• Required: Licensed as a Social Worker from the State Board of Social Work
• Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle

Experience

• Preferred: Minimum one (1) year experience as a social worker in a hospice setting
• Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred

$30.00 - $33.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
