Responsibilities

The Medical Social Worker in hospice provides medical social services under the direction of a physician and Interdisciplinary Group. The role involves assisting in understanding significant social and emotional factors related to the patient's health status and developing coping mechanisms.

• Assist the patient, significant others, physician, and healthcare team to understand personal, emotional, environmental, and social factors affecting health.
• Contribute to the development of a comprehensive, integrated Plan of Care for patients.
• Instruct healthcare team members on community resources available to assist patients.
• Function as Bereavement Coordinator and supervise bereavement services as needed, establishing a plan addressing bereavement needs up to 13 months post-patient death.

Education and Experience

License Requirements:

• Bachelor's Degree in social work from an accredited school or in psychology, sociology, or related field with supervised MSW experience.
• One year of social work experience in a healthcare setting.
• Current CPR certification.
• Valid Driver's License, vehicle insurance, and access to reliable transportation.

Additional State-Specific Requirements:

• Bachelor's degree from a school of Social Work accredited by the Council on Social Work Education.
