Job summary

Agape Hospice - Pinal County is seeking a dedicated Hospice Social Worker to provide psychosocial support to patients with terminal illness and to assist families and caregivers. The role emphasizes compassionate, patient-centered care, documentation, and collaboration with an interdisciplinary team.

The position is per diem in the Pinal County and Casa Grande area, with emphasis on timely assessments and ongoing support for patients and their loved ones within the hospice program.

Qualifications

• Bachelor's degree in social work accredited by CSWE.
• MSW preferred for advanced practice.
• CPR certification required.
• Valid driver’s license and insured vehicle required.

Responsibilities

• Assess psychosocial status of patients and families related to terminal illness and communicate findings to the care team.
• Conduct social evaluations and plan interventions.
• Maintain clinical records for patients referred to social work.
• Provide information and referrals for practical and environmental needs.
• Serve as liaison between patients/families and community agencies.
• Participate in development of the individualized plan of care.

Job Description:

The hospice social worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.

Essential Job Functions/Responsibilities:

• Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicate findings to the registered nurse and other members of the interdisciplinary group
• Carry out social evaluations and plan interventions based on evaluation findings
• Maintain clinical records on all patients referred to social work
• Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs
• Provide information to patients and families/caregivers and community agencies
• Serve as a liaison between patients and families/caregivers and community agencies
• Maintain collaborative relationships with organization personnel to support patient care
• Maintain and develop contracts with public and private agencies as resources for patients and personnel
• Participate in the development of the individualized plan of care and attend regularly scheduled interdisciplinary group meetings
• Assist physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues
• Actively participate in quality assessment performance improvement teams and activities
• Other duties as delegated by the Clinical Supervisor

Position Qualifications:

• A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education; master's program in social work preferred
• Minimum of one year's experience in health care, hospice experience preferred
• Experience in hospice care preferred
• Demonstrates good verbal and written communication, and organization skills
• Possesses and maintains current CPR Certification
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
