Job summary

The ideal candidate has a Master’s in Social Work, an active New Jersey license (LCSW preferred), and at least one year of post-graduate experience in a health setting, with strong commitment to safety, documentation, and compassionate service.

Qualifications

• Master's Degree in Social Work.
• Current NJ social work license; LCSW preferred.
• 1 year of post graduate experience in a health setting.

• Provides direct social work services in the home which meet Agency and professional standards. Meets Agency standards of competency.
• Accurately documents care provided and maintains patient records according to Agency policies.
• Assists with Agency and community programs/committees.
