Job summary

Bayhealth, Inc. is seeking a Per Diem Home Health Medical Social Worker to support patients in their homes, implementing standards of care for medical social work services.

The role emphasizes psychosocial assessment, care planning, and coordination with the clinical team to address patient and family needs and connect them with community resources.

You will maintain clinical records, provide information to patients and families, and participate in team case conferences as required.

Qualifications

• Bachelor's degree in social work accredited by the Council on Social Work Education (CSWE).
• May have a master's or doctoral degree in social work.
• Licensed as a licensed clinical social worker by the Board of Behavioral Sciences.
• Minimum of one year of health care experience.

Responsibilities

• Assesses psychosocial status of patients and communicates to the RN.
• Carries out social evaluations and plans interventions.
• Maintains clinical records for referred patients.
• Provides information and referrals for patients and families.
• Liaises between patients/families and community agencies.

Job description

Bayhealth, Inc. is seeking a Per DiemHome Health Medical Social Worker to serve Bayhealth patients. The Medical Social Worker contracted through the Organization is responsible for the implementation of standards of care for medical social work services.

Responsibilities

• Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.
• Carries out social evaluations and plans intervention based on evaluation findings.
• Maintains clinical records on all patients referred to social work.
• Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.
• Provides information to patients or families/caregivers and community agencies.
• Serves as liaison between patients or families/caregivers and community agencies.
• Maintains collaborative relationships with Organization personnel to support patient care.
• Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.
• Participates in in-service education programs.
• Participates in the development of the total plan of care and case conferences as required.
• Assists physicians and other team members in understanding significant social and emotional factors related to health problems.
• Other duties as delegated by the Clinical Director/Supervisor.

Qualifications

• Graduate of a bachelor’s program in social work accredited by the Council on Social
• Work Education. May have a master’s or doctoral degree in social work.
• Licensed as a licensed clinical social worker by the Board of Behavioral Sciences.
• Minimum of one year\'s experience in health care.
• Experience in home health care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Possesses and maintains current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
