Qualifications

Responsibilities

• Conduct initial and ongoing assessments of residents' social service needs.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Coordinate connections with community resources, home healthcare, hospices, and other services.
• Provide support and counseling to residents and families, addressing concerns and needs.
• Maintain accurate records of resident social service needs and interventions.
• Participate in staff meetings and contribute to policies related to social services.
