Social Worker

General Purpose

To provide the necessary services to ensure that each resident maintains his/her optimal level of psychosocial well-being.

General Purpose

To provide the necessary services to ensure that each resident maintains his/her optimal level of psychosocial well-being.

Community Duties

Essential Duties and Responsibilities:

  • Collaborate with Resident care manager and/or Director to identify resident needs
  • Provide ongoing casework, including but not limited to, interviews, crisis intervention, grief counseling, psychosocial assessments, information and referral, discharge planning, suicide risk assessments and advocacy.
  • Responsible for completing social history and updating resident information
  • Provide one on one counseling for residents/families/staff.
  • Participate in interdisciplinary care planning and complete progress notes or updates
  • Assist residents/families with application and eligibility for programs.
  • Conduct family meetings to prepare resident/family for level of care change.
  • Work as part of the interdisciplinary team to accomplish goals of the department and the organization.
  • Maintain a working knowledge of Medicare/Medicaid/Social security and other benefits.
  • Maintain working knowledge of the Probate court system and the various community- based agencies that could be of benefit to residents.
  • Follows established Infectious Control and Universal Precautions policies and procedures
  • Provide educational seminars/in-services for staff/residents as required. May provide educational presentations for outside groups.
  • Makes home visits to prospective move-ins – for assessment and alignment with appropriate level of care.
  • Participates in quality improvement activities and QAPI.
  • Collaborates with the Assisted Living staff to support transitions of care and ongoing social work needs based on the site of care.

  • Maintains a record of acceptable attendance and punctuality. Observes all Judson rules and safety procedures.

Minimum Qualifications, Education And Certifications

Required Qualifications:

  • Ohio Licensed Social Worker (LSW, BSW,)
  • Computer Literate, with knowledge of Microsoft Office
  • 3 years of experience in a long term, or gerontology setting

Requires good vision, hearing, speech and communication skills with the ability to ambulate, project voice, use the telephone, use the computer and read and write English. Additional physical demands may include kneeling, climbing ladders, reaching, pushing, standing, sitting for long periods of time, and speaking clearly on the telephone for extended periods, crouching, stooping, lifting up to 25 lbs. and repetitive motion.

The fast-paced office-working environment is conducive to be able to computerize essential data, communicate effectively on the telephone and communicate effectively with referrers, caregivers, supervisors, R/Cs and responsible or interested parties. When away from the office, must be within paging and (cell) telephone range to effectively communicate scheduling changes. Will work on the units, in residents’ rooms elsewhere within the building or off site as required. Moves about intermittently throughout the workday with frequent interruptions.
