Job summary

Making Headway Foundation in White Plains, NY is seeking a Social Worker/Family Liaison for full-time, on-site work. The role involves outreach to families, case management, and coordinating with hospitals to secure needed services across the tri-state area.

The position offers salary between $80,000 and $100,000, with medical/dental insurance plans and retirement contributions. Remote/hybrid options may be limited, with some on-site requirements at NYU Hassenfeld partnerships.

Qualifications

• Licensed Social Worker (LMSW or LCSW) with NY state preferred.
• Experience in crisis situations and outreach.
• Proficient in Microsoft Word, PowerPoint and Excel.

Responsibilities

• Perform outreach to newly referred families within 36 hours and coordinate services.
• Provide information, resources, and case management across programs.
• Coordinate with hospital social workers and medical teams for referrals.
• Maintain a detailed database of family interactions and services provided.
• Build partnerships with local nonprofits and hospitals; occasional on-site meetings.
