Reports to: Program Director of Euphrasian Diagnostic Reception Center

Location: 479 New Jersey Ave, Brooklyn, NY 11207

Hours: 35 Hours/Week; Exempt (Position Requires Staff to be on site. Two Evenings Required)

Additional Compensation: $1,000 sign-on bonus paid in first payroll and $2,000 retention bonus paid out after first year of employment.

Job Summary: The Social Worker Coordinator plays a vital role in the Shirley Chisholm Youth Reception Center Social Service team. The coordinator collaborates with an inter-disciplinary team to assess treatment needs, plan and coordinate services, and provide short-term interventions. The Social Services Coordinator embodies Good Shepherd Services’ mission, demonstrating our commitment that “We See What Can Be” in our approach to participants.

Major Duties:

  • Perform intake and admissions activities collaboratively with the Case Worker, Residential Life, and Health Services.
  • Identify permanent connections and develop discharge plans during admissions, including using genograms.
  • Conduct psycho-social evaluations and coordinate assessments by other providers, including psychiatrists and psychologists.
  • Coordinate treatment service planning and facilitate Treatment Team Meetings, treatment plan development, and monitoring.
  • Work with the treatment team and Office of Placement Services to expedite participant placement or discharge, interfacing with recommended placements.
  • Explore alternative resources with the Child Protective Specialist/Case Planner to facilitate replacement or discharge, including kinship resources.
  • For long-term participants, initiate family team meetings to develop strategies for replacement or permanency.
  • Coordinate mental health activities with Health Services, including obtaining mental health histories and facilitating referrals.
  • Coordinate post-crisis responses, including crisis intervention and avoidance services.
  • Provide short-term individual and family therapy where appropriate and support behavioral management interventions.
  • Facilitate psychoeducation groups with participants.
  • Promote permanent connections such as reunification, foster placement, or adoption.
  • Participate in supervision, trainings, and meetings as required.
  • Maintain accurate mandated paperwork and records in compliance with regulations.
  • Utilize Sanctuary Model tools to foster a trauma-sensitive culture, including safety plans and community meetings.
  • Model and promote Sanctuary Model commitments to ensure a non-violent, trauma-informed environment.
  • Integrate the Sanctuary Model SELF framework into daily practice for quality care and a positive work environment.
  • Perform other duties as assigned.

Qualifications:

  • NYS Licensed Masters or Clinical Social Worker (LMSW or LCSW), with at least two (2) years in child welfare or related settings.
  • Master's degree in Social Work required.
  • Experience in clinical work with youth and families, including crisis management.
  • Strong participant sensitivity, communication skills, and team facilitation experience.
  • Excellent verbal, written, organizational, and computer skills.
  • Ability to work independently with initiative and as part of a team.
  • Must be able to use public transportation; a NY State Driver’s license is preferred.
  • Excellent crisis intervention, advocacy, and problem-solving skills.
  • Flexibility to work evenings, weekends, holidays, and be on-call as needed.
  • Commitment to a strength-based, youth development, and trauma-informed approach.
