The Social Worker/Counselor provides direct support to patients: treatment planning, therapeutic group services, individual therapy, discharge planning, and case management. Works with a professional team of behavioral health staff to provide excellent care and customer service to patients served.

The Social Worker/Counselor provides direct support to patients: treatment planning, therapeutic group services, individual therapy, discharge planning, and case management. Works with a professional team of behavioral health staff to provide excellent care and customer service to patients served.

Responsibilities

  • Prepares intake screening interviews and psychosocial assessments and diagnostic formulations for use in treatment and discharge planning, and Level of Care (LOC) considerations. Confers with members of the professional clinical staff throughout all patient treatment processes.
  • Evaluates and responds to any urgent crisis of patient and/or their families. Assess needs for care, provide direct crisis counseling and take immediate steps to stabilize clients in acute distress.
  • MHU- Consult and confer with physicians and nurses regarding all psych-social aspects of patient care including interventions, discharge planning and treatment referrals.
  • Directly engages patients in a variety of settings including, but not limited to, individual therapy, group therapy, activity therapy and recreational therapy. Plans and facilitates therapeutic group services. Utilizes motivational techniques for patient participation and engagement in therapeutic milieu.
  • ED- Facilitates referrals needed, i.e.: admission to the Mental Health Unit, referral for detoxification services and all other follow-up care. Make contact with referral source and provide any needed information to ensure the best outcome for the patient; MHU-. Refers patients to appropriate services with complete instructions for follow up care, and makes contact with referral source to provide relevant clinical information to ensure the best outcome for the patient.
  • ED- Completes utilization reviews by providing all relevant clinical information to insurance companies for any pre-certification of hospitalization; MHU- Assists Psychiatric UR staff with pre-cert and continued stay utilization reviews as needed.
  • Provides patients and families with education, comfort, and relevant information in accordance with their care as needed within the facility and throughout the community.
  • All other duties as assigned by department.

Qualifications

Master's Degree in Social Work (Clinical) and/or Master's Degree Marriage & Family and/or Master’s Degree Mental Health/Mental Health Counseling

2 years in Clinical Mental Health Setting is required.

Florida license in Clinical Social Work, Marriage and Family Therapy, or Mental Health Counseling

or

Florida license in Registered Mental Health Counselor Intern or Registered Clinical Social Worker Intern

Registered Mental Health Counselor Intern or Registered Clinical Social Worker Intern license is required to obtain FL license for Clinical Social Worker, or Marriage and Family Therapist, or Mental Health Counselor within 2 years of employment.

Level 2 DCF Background Screening required upon hire.

BLS, provided by American Heart Association (AHA), required within 30 days of hire.

Completion of WELLE training (Non-Abusive Psychological & Physical Intervention) within 90-days of hire.
