Qualifications

• Bachelor's degree in Social Work or related field.
• Illinois Social Work license is preferred.
• Experience in skilled nursing, long-term care, healthcare, or social services is preferred.

Responsibilities

• Provide social services support to residents and families.
• Assess psychosocial needs and develop individualized care plans.
• Provide emotional support, counseling, and crisis intervention as needed.
• Collaborate with nursing and other departments in care planning.
• Coordinate discharge planning and transitions to appropriate care levels.
• Advocate for resident rights, privacy, and quality of life.
• Maintain documentation per facility policies and regulatory requirements.
