Job summary

Always Best Care Senior Services - O'ahu is seeking a Medical Social Worker (MSW) to join our home health team. You will support patients and families dealing with emotional, financial, and practical challenges in the comfort of their homes.

Responsibilities include psychosocial assessment, counseling, crisis intervention, connecting clients with community resources, supporting discharge planning, coordinating with the care team, and maintaining accurate documentation.

Qualifications

• Master's degree in social work.
• Current Hawaii LSW license that meets home health medical social services requirements.
• At least one year of experience in healthcare, home health, or social services preferred.
• Knowledge of community resources and social service programs.
• Strong assessment, counseling, communication, documentation, advocacy, and organizational skills.
• Valid drivers' license, reliable transportation, and automobile insurance when required by agency policy.

Responsibilities

• Assess patients' psychosocial and emotional needs, family dynamics, home environment, and financial concerns.
• Provide counseling, emotional support, crisis intervention, advocacy, and education.
• Connect patients and caregivers with community resources, including financial, housing, transportation, legal, and long-term care services.
• Support discharge planning and transitions of care.
• Coordinate with physicians, nurses, therapists, and the rest of the care team, and take part in case conferences.
• Keep accurate, timely assessments, visit notes, and progress documentation.
• Take part in quality improvement (QAPI) activities.
