Job description

The Case Manager (CM), reporting to the Lead Case Manager will work directly with clients and their family/support system of various diverse socio-economic backgrounds to establish rapport while facilitating wellness around complex physical and/or mental health conditions to improve quality of life. The CM will be cross-trained in all case management, transition, and diversion programs to cover if needed. The CM works as a member of the Choice in Aging (CiA) team providing health and social services to frail elders and adults with disabilities to meet CiA’s mission to create opportunities where people can learn, grow and age independently with dignity and community.

Essential Duties:

  • Work with older adults in Contra Costa County to help improve their quality of life by developing care plans, facilitating access to resources, advocating for client rights, and promoting self-sufficiency and empowerment.
  • Locate, contact, and engage members who have been identified as candidates using a progressive approach.
  • Document outreach and engagement attempts and modalities.
  • Identify necessary clinical and non-clinical resources that may be needed to address gaps in care.
  • Develop a person-centered care plan leveraging participant strengths, goals, and preferences.

Minimum Qualifications:
  • Bachelor’s level degree required, experience in case management with older and/or disabled adults; nonprofit experience preferred.
  • Working knowledge of MS office, Google suite, and basic internet usage.
  • Experience working with the Older Adult population preferred.
  • Bi-lingual English/Spanish or English/Tagalog desired.

Job Type: Full-Time
Monday-Friday (Closed on all major holidays)

Hourly Rate: $26.44 - $28.84
Staff working Full Time (30+ hours per week) & Part Time (20+ hours per week)
