Job description

Shift:

A minimum of three 8-hour shifts are required within a 6-week schedule period. 16 of the 24 hours must be weekend shifts. Occasional holiday shifts are also required. Shifts are from 7:30 AM to 4:00 PM.

GENERAL SUMMARY:

Coordinates the overall interdisciplinary plan of care for the patient, from admission to discharge.

Responsibilities:
  1. Creates a care plan across the continuum, integrating patient and family preferences and values.
  2. Serves as a resource to assess, evaluate, recommend, and treat legal, neglect, substance abuse, crisis, psychosocial, and various adjustment needs.
  3. Advocates for resources and removal of barriers.
  4. Maintains ongoing communication with supervisors and care transition team members to ensure effective implementation and reevaluation of the health plan.
  5. Acts as a resource for proper medical record documentation, complex level of care recommendations, and services related to diagnoses and treatment options for post-discharge care.
CERTIFICATION/LICENCES REQUIRED:
  • One or more of the following are required:
    • Counselor credentialed from the Michigan Board of Counseling obtained prior to hire or transfer date.
    • Social Worker credentialed from the Michigan Board of Social Work obtained prior to hire or transfer date. Any level accepted.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) is preferred.
EDUCATION/EXPERIENCE REQUIRED:
  • Bachelor's degree required.
  • Master's degree preferred.
  • Behavioral Health experience preferred.
  • Prior acute care experience preferred.
  • Experience with Careport/ECIN/Allscripts preferred.
  • Experience with Outlook/Google preferred.
