Job summary

Your Health in Brunswick is seeking a qualified Social Worker to assist patients across the Brunswick area, visiting homes and care facilities. Reliable transportation is required daily, and the role is full-time, 8‑hour shifts from 8:00 AM to 5:00 PM, Monday through Friday.

The Social Worker will help with advance care planning, cognitive assessments, placement, and general resources, coordinating with care teams and facilities to ensure compassionate, coordinated care for patients in ALF and

Responsibilities

• Initiating Advance Care Planning for patients throughout their assigned Care Team (including facilities).
• Completing Daily Comprehensive Assessments on patients.
• Completing Daily Care Plans on patients.
• Documenting Chronic Care Management time daily.
• Meeting Daily Productivity Goals
• Coordinating discharges at contracted facilities
• Coordinating Placement.
• Continuation of coverage (i.e. coordinating acute visits for falls, med changes, etc.)
• Build and maintain professional relationships with referral sources.
• Maintain strong communication and professionalism between referral sources, patients/families, and Care Teams.
