Job summary

Your Health is actively seeking a dedicated Social Worker to support patients in the Savannah area, assisting with advance care planning, assessments, placement, and resources across multiple facilities and homes. Travel to homes and facilities is required daily, with 8-hour shifts from 8am-5pm, Monday to Friday, in a full-time role within our physician group.

Qualifications

• Compassionate, patient-centered service approach.
• Excellent communication and coordination with care teams.

Responsibilities

• Initiate Advance Care Planning for patients across care teams.
• Complete Daily Comprehensive Assessments.
• Develop Daily Care Plans.
• Document Chronic Care Management activities.
• Meet Daily Productivity goals.
• Coordinate discharges at facilities and placement.
• Maintain relationships with referral sources and families.
