Responsibilities

The Social Worker collaborates with the clinical team to provide patient and family advocacy, discharge planning, and psychosocial interventions. The goal is to promote wellness, improve care outcomes, and facilitate access to hospital and community resources. The role may also include crisis intervention and legal reporting as needed.

Qualifications
  • Required Education: Graduation from an accredited School of Social Work; Master’s degree in Social Work; current LMSW license.
  • Preferred Education: CCM Certification.
  • Required Skills: Basic computer skills and keyboarding.
  • Preferred Skills: Experience with Meditech Electronic Medical Record system.
  • Required Experience: At least three years of social work experience.
  • Preferred Experience: Experience working in a hospital setting.
