Job summary

HireCare Services – Massachusetts seeks a compassionate Social Worker to support residents and families in a skilled nursing/rehab setting at Woburn Rehabilitation and Nursing Center. You will conduct psychosocial assessments, assist with discharge planning, and coordinate care to help residents thrive during their stay.

The role emphasizes collaboration with the interdisciplinary team, strong documentation, and adherence to state and federal guidelines.

Qualifications

• BSW required (MSW preferred)
• Experience in SNF/LTC/Rehab strongly preferred
• Strong communication and documentation skills
• Familiarity with discharge planning, community resources, and resident rights
• Organized, empathetic, and able to manage multiple priorities

Responsibilities

• Complete psychosocial assessments and contribute to individualized care plans
• Provide support/counseling to residents and families and assist with adjustment to the facility
• Lead and support discharge planning (coordination with hospitals, families, and community resources)
• Collaborate with nursing, therapy, and the interdisciplinary team
• Assist with admissions, transfers, and resident/family communication
• Ensure timely, accurate documentation and compliance with state/federal guidelines
• Participate in care plan meetings and follow up on assigned action items
