Job Summary

Carease Health - PA, a leading provider of comprehensive preventative care programs, is seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team as a Social Worker/Care Companion in Murrysville, PA. In this part-time role, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

Key Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor to conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Document visit notes and maintain detailed records of interactions and interventions after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications
